Mansfield Fire Rescue processes requests for reports through the City Secretary’s Office. Requests for reports must be made in writing and are accepted for processing 72 hours after the incident. You may email your request to Open Records.
Request for EMS records must be made in person at the Fire Administration office. Individuals may request personal EMS records, their minor children’s EMS records, or a deceased family member EMS records. EMS records for deceased family members must be next-of-kin. Individuals must show proof of identification at the time of the request. Anyone who requests EMS records that are not their own must present a HIPAA compliant medical authorization to release form signed by the patient named in the record.
